Who We Are
Grace Counseling LLC, doing business as Strengthen Social Skills, is a licensed mental health practice located at 20 Northdale Blvd NW, Coon Rapids, MN 55448. We are required by law to maintain the privacy of your health information, to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of this Notice.
How We May Use and Disclose Your Health Information
We use and disclose health information for the following purposes:
Treatment
We may use and share your health information to provide you with clinical services. For example, your clinician may share relevant information with other members of your care team.
Payment
We may use and share your health information to bill and receive payment for the services we provide. For example, we may send information to your health insurance company to obtain payment for services.
Health Care Operations
We may use and share your health information for internal operations such as quality improvement, staff training, supervision, and business administration.
Required by Law
We will share information about you if required to do so by federal, state, or local law, including mandatory reporting obligations.
Public Health and Safety
We may share your information in situations involving imminent risk of serious harm to you or others, as permitted or required by applicable law.
Uses and Disclosures Requiring Your Authorization
Other uses and disclosures of your health information will be made only with your written authorization, including:
- Psychotherapy notes (except in limited circumstances)
- Marketing communications
- Sale of health information
- Most other uses and disclosures not described in this Notice
You may revoke your authorization in writing at any time, except to the extent that we have already acted on it.
Your Rights
Right to Access Your Records
You have the right to inspect and obtain a copy of your health information. Requests must be made in writing. We may charge a reasonable fee for copies.
Right to Request Amendment
You have the right to request that we amend your health information if you believe it is incorrect or incomplete. We may deny your request under certain circumstances.
Right to an Accounting of Disclosures
You have the right to request a list of disclosures we have made of your health information in the past six years, other than disclosures for treatment, payment, or health care operations.
Right to Request Restrictions
You have the right to request restrictions on how we use or disclose your health information. We are not required to agree to all restrictions, but if we agree, we will comply with your request except in emergency situations.
Right to Confidential Communications
You have the right to request that we communicate with you by alternative means or at an alternative location (for example, by email rather than by mail). We will accommodate reasonable requests.
Right to a Paper Copy of This Notice
You have the right to a paper copy of this Notice at any time, even if you have received it electronically.
Right to Be Notified of a Breach
You have the right to be notified if there is a breach of your unsecured protected health information.
Our Duties
We are required by law to:
- Maintain the privacy of your health information
- Provide you with this Notice of our legal duties and privacy practices
- Follow the terms of the Notice currently in effect
- Notify you in the event of a breach of your unsecured protected health information
We reserve the right to change our privacy practices and to make the new practices effective for all health information we maintain. If we make a material change to our privacy practices, we will post the revised Notice on our website.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. To file a complaint with us, contact our Privacy Officer:
